Valley Medical and Wellness is a premier chronic pain medicine and addiction medicine practice with offices in Burnsville, Minneapolis, Woodbury, Rochester, and Bemidji Minnesota. Our mission is to help people get back to loving life.
At all five of our clinics we strive to improve the quality of life for those suffering from pain and/or addiction. We employ a comprehensive approach that focuses on overall wellness along with progressive and knowledgeable medical treatments. We work closely with our patients and we recognize that dealing with pain and/or addiction can impact every aspect of your lives, including your self-worth, physical health, jobs, relationships with friends and family, finances, and mental health.
Under the guidance of Svetlana Zaydman, DO, and Ashwin George, MD, our patients receive customized treatment that identifies their pain or addiction problem at its source. Our treatment goals and puts a plan into action that helps achieve these goals. We are a member of the Fairview Physicians Association (FPA) and are able to refer out a full range of medical services.
Pain Management
Our pain management program focuses on improving daily function and quality of life by reducing pain and dependence to opioids. Pain management strategies are personalized to each patient and incorporate exercise, nutritional habits, and interventional pain procedures to help further improve function. Addiction Treatment
Our addiction treatment program provides medication-assisted treatment (MAT) for using proven medications like Buprenorphine (e.g. Suboxone, Sublocade) or Naltrexone (e.g. Vivitrol) to help stabilize cravings and withdrawals. This helps reduce dependence on opiates (e.g. prescription pain pills, heroin, fentanyl), stimulants (e.g. cocaine, methamphetamines), benzodiazepines (e.g. Xanax), marijuana, and alcohol. In addition, our providers perform mental health and wellness screenings to understand the overall health of the patient.
We are also uniquely able to treat a patient’s pain, which is a common trigger for patients with substance use disorder. When counseling is needed for substance abuse, we partner with licensed counselors within the community and across the state for comprehensive assessments, individual and group therapy, and patient education.
STI Testing and Treatment
VMW also offers testing for the most common sexually transmitted infections (STIs) and women's sexual wellness issues, including fungal infections. Our STI testing is fast and completely confidential and we can prescribe treatment via a remote telemedicine visit.
COVID-19 Testing and Treatment
While we remain committed to fighting the opioid epidemic, VMW is also helping to fight the COVID-19 (coronavirus) pandemic by offering drive-thru community RT-PCR COVID-19 and influenza testing. Since COVID and the flu share many symptoms, we're able to offer a single-swab procedure that tests for both. If you test positive, we can prescribe Paxlovid (for COVID) or Tamiflu (for influenza) via a convenient telehealth appointment. We also serve as a COVID-19 testing referral lab for other providers.

Q: Can I schedule an appointment at any location, or am I restricted to the location or provider I scheduled at first?
A: You are welcome to see any provider at any one of our locations at your convenience.
Q: How do I schedule an appointment with Valley Medical and Wellness?
A: For existing patients, you can call the front desk or message us on Klara. New Patients looking to establish care with VMW should call our new patient scheduling line at 612-444-3000 option 1, or click the “message us” button at the bottom of the page. You may also have your doctor send us a referral and we’ll call you! You can complete our new patient forms on our website HERE.
Q: How do I request my medical records?
A: If you would like your medical records sent to an outside facility, complete our Release of Information form HERE and send an email with your request to patientservices@valleymedical.com.
Q: What type of insurance does VMW accept?
A: Valley Medical is in-network with most major insurance companies, including Blue Cross, Health Partners, Medica, Medicare, Minnesota Medicaid, Preferred One, UCare, and United Healthcare. We are also a member of the VA's Community Care Network and take TRICARE for active and retired military personnel and thier families. Q: Does VMW prescribe medical cannabis (THC)?
A: While Valley Medical does provide some medical cannabis certifications, this is not a primary treatment option for pain management within our clinic. Medical cannabis certifications are offered as part of our larger treatment program for established patients as an adjunct therapy. Valley Medical does not typically offer medical cannabis certifications to new patients or as a primary treatment option.
Q: What type of services does VMW offer?
A: Valley Medical and Wellness is a premium Chronic Pain Management and Addiction Management specialty clinic. COVID-19 and Flu Testing and Protocols
Q: Can I get tested for COVID-19 and the flu at VMW?
A: Yes. Valley Medical and Wellness (VMW) offers COVID-19 and flu testing for both our existing VMW patients as well as their families and anyone else needing a test for any reason. RT-PCR nasal swab tests are available Monday – Friday in Burnsville, Minneapolis, and Woodbury with same-day results.
Q: Is VMW staying open during the COVID-19 pandemic?
A: Valley Medical and Wellness (VMW) has remained open throughout the pandemic. We continue to see patients as normal and our hours of operation continue to be the same for each location. VMW also has a COVID-19 Testing Laboratory and is able to provide COVID-19 Testing for both our existing VMW patients as well as their families and anyone else needing a test for any reason.
Q: What is VMW doing to protect me during my office visit?
A: Valley Medical and Wellness (VMW) is taking the COVID-19 pandemic very seriously. Your health is our number one priority and we are committed to helping stop the spread of the virus and to protecting our patients, staff, and the community. Although these are unsettling times and your daily experience may continue to change, our team is dedicated to continuing to serve our patients every day.
We are taking additional precautions by implementing more rigorous disinfecting protocols between VMW staff and our contracted cleaning services. This includes disinfecting all common areas multiple times per day such as the credit card machine, beverage stands, and all communal surfaces such as chairs and desks. All patients and their essential caregivers and all staff members are required to wear a mask while in the building in addition to providing daily screenings.
In response to the COVID-19 pandemic, VMW offers COVID-19 Testing for both our existing VMW patients as well as their families and anyone else needing a test for any reason. We offer testing in Burnsville, Woodbury, and Minneapolis with same-day results. These tests will be conducted outside of the clinic so there is no additional risk to our current patients receiving our other services.

COVID-19 for Existing VMW Patients
Q: What if I am self-quarantined and cannot leave my home?
A: If you are self-quarantined because you have come into contact with someone who is ill; you are experiencing symptoms such as cough, sore throat, runny nose, or fever, VMW is now offering "Curbside Visits" in order to help minimize exposure to our patients and staff. Patients who are under quarantine are still allowed to leave for medical appointments, and your appointment confirmation texts or Klara can be used to verify your appointment as needed. These visits are conducted entirely from your vehicle and are available to our current patients.
Q: Can I still have my office visit if I do have cold- or flu-like symptoms?
A: If you are experiencing cold- or flu-like symptoms, we ask that you do not come into the clinic to risk exposing others to illness. VMW is now offering curbside visits and your entire appointment will be conducted from your vehicle. If you are too sick to come for a curbside appointment, we can reschedule your appointment until you are feeling well enough.

Q: What can I expect from my "Curbside Visit?"
A: For our existing VMW patients (not related to Drive-Thru COVID Testing), Curbside Visits will be available at our Burnsville and Woodbury locations for our current patients who are symptomatic. They are done entirely from your vehicle so that we can minimize the contact you have with others. If you are needing a Curbside Visit, please message scheduling via Klara or call us at 612-444-3000 OPTION 2.
Once you arrive for your scheduled Curbside Visit, do NOT exit your vehicle. Please contact the front desk and alert them of your arrival and "check in" for your appointment over the phone and pay necessary copays by dialing 612-444-3000 OPTION 2. Our medical staff will guide you through a series of questions on the phone and then your provider will meet you at your vehicle to conduct your visit and collect your urine specimen. You are required to bring a urine specimen with you to these appointments which can be brought in any sealable container such as an empty, clean water bottle or a urine specimen cup, which can be purchased at most pharmacies.
Q: What if I have tested positive for COVID-19?
A: If you have tested positive for COVID-19, we ask that you stay home and follow the CDC and Minnesota Department of Health guidelines. We will ensure you will be able to continue your currently prescribed medication during your quarantine period. We will address this on a case-by-case basis and can offer you a Curbside or Telemedicine visit during your quarantine period.
Q: Can I get my medication refilled without an office visit?
A: Unfortunately, we cannot send in prescriptions for controlled substance refills without an office visit. However, if you are sick, we can offer you accelerated "Curbside Visits" entirely from your vehicle.
Q: Do I need to wear a mask when I come to my appointment?
A: In accordance with CDC Guidelines all patients are required to wear a mask as of 6/15/2020. If you are symptomatic, we ask that you please let the clinic know at least 48 hours prior to your appointment so that we may reschedule your appointment or offer you a "curbside visit".
Q: My primary doctor is doing appointments over video, does VMW have the option for telemedicine visits?
A: VMW offers telemedicine visits to individuals who are symptomatic of COVID-19 and cannot obtain private transportation to have a curbside visit and for those who are quarantined due to a positive COVID-19 test. These appointments are scheduled on a case by case basis and are available to current patients.
